• The doctor, hospital or service page from which you started the enquiry, when applicable.

How information is used

Enquiry information is stored so DM Flymeds can respond to the request and manage the enquiry. It is not intended for diagnosis or detailed medical records.

What not to submit

Please do not submit detailed medical histories, prescriptions, test reports, government identification numbers or payment information through this form.

Retention and contact

A production retention period and privacy contact must be configured by the business before launch. Until then, this site should be treated as a product demonstration.
